So yeah, pretty much, I'm trying to narrow down what actually is making the difference in cursor lag when various DirectX based games and applications is so much worse in windowed mode as opposed to fullscreen mode.

Now, here are a few tests I've done to try and resolve this issue which only appears to be present in Vista and Windows 7 and 8.

  • Disable desktop composition
  • Disable visual themes and switch to classic
  • Change to a lesser window size
  • Kill most processes and try various processing priorities/thread priorities
  • Use a bitmap desktop background/no background
  • Kill explorer.exe and use no shell

So far, I have not found the cure for this, and the thing that gives me hope, is the fact this issue was and is not present in any version of Windows prior to Vista.

The game I've noticed this most in and been basing these tests on is osu!

I know some of you here have vast knowledge in areas like this, I would greatly appreciate it if someone could help diagnose the cause of this issue.

Might I suggest installing VMware or VirtualBox and a Windows 98SE or XP VM? I always keep a 98SE and an XP VM on my computer for those games that are too new for DOSbox but too old to work properly on newer OSes :) Probably that would be your best bet for running PC games circa 1996-2003...

I noticed that something else using hardware accelerated graphics like a web browser can cause negative performance too (I seen my GPU actually sometimes use a lower clock in that situation). But seeing you listed "kill most processes" as a step, I'd assume you indirectly tried that.

Yep, unfortunately.
Mind you, this isn't just with older games and apps, this is with both old ones and current ones. :/
