I honestly have never seen a single episode of Dr. Who, all I know about it comes from being around people who were huge fans

But yeah- to the point where it remains the police box today, despite the fact that apparently England doesn't have them anymore. (I say England because I think the Scottish ones looked different or something?)

100% fact.

In one episode, it was fixed... but not quite, and the disguises were oddly chosen -- a pipe organ, for example. For functional TARDISes without an obvious entry point, they always just snuck behind the back.

In one recent episode (The Doctor's Wife), the Doctor built a barebones TARDIS to chase after his own, which obviously didn't include a chameleon circuit, or even the transdimensionality -- it was exactly as big as it seemed to be.

The only thing about the TARDIS that regularly changed was the control room. ("I see you changed the desktop wallpaper? I don't like it.") That same episode I just mentioned had all the old control rooms archived for backup, though they only went back one -- the coral-theme room from the Ninth and Tenth Doctors, which had originally burned down when Ten rather explosively regenerated into Eleven.
